Dental Benefits


Dental benefits are provided through a third-party administrator, Sele-dent, Inc. You may
choose to use a participating provider in the Sele-dent network of dentists, or choose a non-
participating provider.

Question: What is the dental deductible?

Answer: Under the Sele-Dent plan, there is a $50.00 deductible per person, up to a maximum
of $150.00 per family, per calendar year. This deductible does not apply if you use a Sele-Dent
PPO dentist.

Question: If I use a PPO dentist, do I have to pay anything?

Answer: No. Preventive, basic, and major services are all covered at 100% of the discounted
fee, up to your annual maximum of $2,000.00.

Question: Is there a website I can use to search for Dental providers in my area?

Answer: Yes. Use the Sele-Dent website. It is free and easy to use. Click on the link below to
try it.

Question: Are dental implants covered?

Answer: No. Dental implants are considered cosmetic and are not a covered service under
our dental plan.

Question: Can I go to any dentist?

Answer: Yes. However, if you choose to use a dentist who does not participate with the Sele-
Dent PPO, you may incur some out-of-pocket expense, as you will be subject to the annual
deductible amounts mentioned above. Also, the coinsurance amount changes when using a
non- participating dentist. Although Preventive care will remain payable at 100% of reasonable
and customary charges, Basic care will be payable at 80%, and Major care will be payable at
50% of reasonable and customary.

Question: Does the plan have an annual maximum amount payable per year?

Answer: Yes. The annual maximum amount payable is $2,000.00 per family member.

Question: Do we have coverage for braces?

Answer: Yes. You have a lifetime orthodontia benefit of $2,000.00 per family member.
Benefits are payable at 50% until you reach your lifetime orthodontia maximum. Orthodontia
payments are not applied to your annual benefit maximum mentioned above.

benefit information as this plan is not administered by the Fund Office.
